I noticed you said "Not every desire is conscious..." That's important. Many desires are 'implanted' by family and society. It takes a 'vision quest', a true soul search, to understand our conscious and subconscious desires, and desires versus passion.
Further, taking a personality test (http://www.keirsey.com/) may reveal your temperament over the years, to see if being a clinical physician will fulfill your life.

I worked as a data scientist for Syndero (top ten fastest growth startup in SF in 2008) a few years back... but felt empty analyzing and datamining sales and advertising data. So I quit and have been doing my own startup in biomedical datamining (bzillion.com and gogenes.org).

There are 8 medical doctors among my immediate relatives, heart surgeon, ophthalmologist, radiologist, psychiatrist, and others. But I don't find any of them inspiring. In fact their knowledge are very specific and narrowly focus. And nothing they do are patent-able. They are providers, fixers (surgeons), and perhaps healers.

I found medical informatics much more interesting and fit my personality as a researcher, datamining scientist. I filed a patent for my invention in curating biomedical papers. There are options. Keep meditating on it.

Best wishes.

Bing....
I completely understand. I am in a similar place... I am 50 yrs old, i have raised 4 children, i am prior service, USAF, and i listened to the voice inside, re enlisted in the military..... 3 yrs ago, when i was laid off from my technical job, i decided to do something i have always wanted to do... become an EMT...not a job that pays much... but, it was my inner voice...i wanted to do this. Now, i want to become a flight medic, or field medic... military trained....
i am jumping through hoops... they will not make it easy for me.... i am up for the challenge... i will be tested at every turn, and believe me, i am sure they will disallow it, if they can.... but, right now, they can't. so.... i am following my dream. it will not be easy, it will not be the most enjoyable journey, but it is what, ...as you.... i need to do... i have no choice... it is what my heart , my soul, needs to do.
if it meant to be, in our highest good, on our highest path and purpose for this lifetime, we will succeed.....
i send you my support.... godspeed.... off we go....
sincerely
